Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient forms. Completing the forms prior to your appointment will allow us to attend to your dental needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards as well as Care Credit. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Financial Policy

Payment is due the same day as services are rendered unless prior financial arrangements have been made.  To make treatment more accessible for patients we now currently offer Care Credit financing which increases access to care for many people. In addition, we accept all major credit cards. 

Dental Insurance

For the convenience of our patients we accept all dental PPO plans as well as participate with the Maryland Healthy Smiles plan for children up to 21 years old and pregnant patients.  As a courtesy to our patients we offer insurance verification and benefit information upon request.
